    Programs and Services

    The Family Health Team provides primary health care services to its patients with the following foci:

    » Integrating a broad range of allied health professionals into primary care.
    » Providing a comprehensive program of patient education designed to support patients better manage their own care.
    » Providing information and leadership to patients on wellness and prevention.
    » Supporting patients to better manage chronic disease such as heart disease and diabetes.


    The Minto-Mapleton Family Health Team is currently developing targeted services and programs in four specific areas:

    » Mental Health.
    » Chronic Disease Management with a targeted focus on heart disease, diabetes and obesity.
    » Health Promotion and Disease Prevention with a specific focus on physical activity/exercise, nutrition, smoking cessation and stress reduction/management.
    » Accessible services for our Mennonite patient population, including Low German Speaking Mennonites from Mexico.


    Services Include:

    Geriatric Psychiatry - Dr. Crowson
    The Minto-Mapleton Family Health Team offers Geriatric Psychiatry Services via video conferencing from our Clifford and Drayton locations.

    Dr. Crowson is a Psychiatrist from St. Joseph's Healthcare, Hamilton working with our Registered Nurse, Pam Turton, to provide assessments for patients 65 years and older, or for any age-related concerns with any of the following criteria:

    >       Behavioural difficulties
    >       Cognitive decline / Memory loss / Dementia
    >       Mood disorder, anxiety, psychosis
    >       Psychiatric medication review
    >       Substance misuse.

    Follow up is provided as needed.

    Memory Clinic
    The Minto-Mapleton Family Health Team offers a Memory Clinic monthly at our Drayton site. It is a multi-disciplinary team led by Dr. Donald and includes the Registered Nurse, Social Worker and Dispensary staff.

    This clinic provides team based assessment and follow-up using specific diagnostic tools and investigative tests for patients with cognitive impairments. Patients are to attend the clinic accompanied by their spouse or a family member. Caregiver support and linkages to the community will be provided as well.

    Chronic Obstructive Pulmonary Disease Program
    Perhaps you or someone you know is living with breathing problems. The Minto-Mapleton Family Health Team is offering new programs to help our patients with Chronic Obstructive Pulmonary Disease (COPD).

    COPD is the term for lung conditions such as chronic bronchitis or emphysema. When a person suffers from COPD the damage done to the tissue of the lungs makes breathing a chore.

    The COPD program offers a group workshop, Living Well with COPD and teaches self-management and coping skills. The Nurse Practitioner, Pharmacist, Registered Nurse, Dietitian and Health Promoter/Exercise Specialist make up the multi-disciplinary team in place to assist our patients improve their quality of life while dealing with lung conditions.

    This program has partnered with St-Mary's Hospital in Kitchener to offer video conferencing with their Respirologist department. This would allow our patients access to lung specialist from our Clifford or Drayton office.

    Activity Counselling
    > Wanting to start a regular exercise program?
    > Don't know where to start?
    > Exercise doesn't have to hurt to be effective!

    Adding more regular exercise to your life will give you increased energy, make you feel better and improve your quality of life. Regardless of what stage you are at with your exercise program, our Exercise Specialist can help you determine your current fitness level and create an individualized program aimed at improving your health.

    Some of the concerns our health promoter can help you learn more about include:

    »       Manage your diabetes
    »       Decrease pain
    »       Reduce stress

    Exercise programs can also be tailored to those with chronic disease or other health problems.

    Pharmacy Counselling
    Our clinical pharmacist helps our patients better understand what medications they are taking and what they do. The pharmacist works closely with the primary health care provider to make decisions about medication choices, drug interactions and the best way to help our patients, if medication is an option.

    The pharmacist can also assist with medication organization and management. By helping our patients understand how food, activity and other medications affect their medication, patients become more knowledgeable.

    Nutrition Counselling

    Our registered dietitian can help you with many nutrition related concerns. Healthy eating is an important part of overall well-being. Whether you have a specific medical condition or just want more information about choosing healthier foods, our dietitian can help you make informed choices about the food you eat.

    Heart to Heart
    What is Heart to Heart?
    Heart to Heart is a six-session program of education and support for cardiac patients and their partners. It was developed by the Heart and Stroke Foundation to meet the needs of people learning to live with heart disease and risk factors associated with this process.

    What happens at the meetings?
    Each two hour session, you and your partner will discover how to live with heart disease and how to make important lifestyle changes.
    Videos or guest speakers and informal group discussions are used to provide information. You'll have opportunities to ask questions, share common concerns with group members and take the next step on the road to recovery.
    Heart to Heart groups are lead by a knowledgeable and caring facilitator who'll help you identify solutions and offer practical suggestions.

    Is this program right for me?
    If you or someone you care about has had a heart attack, heart surgery, have a number of risk factors for heart disease or suffering from angina, Heart to Heart gives you an opportunity to understand what's happened and get your questions answered.
    Knowing what to expect and learning what to do to lead as healthy a life as possible will start you and your partner on the road to recovery more quickly and easily.

    Should my partner attend?
    Yes! A partner, or family member, should attend with you - they usually have questions too.
    Your partner will welcome the opportunity to talk with others and learn how they're dealing with similar concerns. By working together, you'll both be better able to face the challenges that living with heart disease brings.

    Does my doctor know about the program?
    Information about the program is given to family doctors and cardiologists. Many encourage their partners to attend.
    Your doctor and the other health professionals directly involved in your care are the best people to answer your questions. Heart to Heart provides you with information you can discuss with them.

    Mental Health/Counselling ServicesThe Family Health Team currently has two social workers who offer psycho-education related to the prevention of mental health problems as well as individual, couple, family, and groupcounselling and psychotherapy.

    Specific mental health concerns that the social workers can help with include:

    >       Self-Esteem
    >       Stress Management
    >       Parenting Concerns
    >       Coping with Illness
    >       Couple and Family Problems
    >       Grief

    Smoking Cessation Program
    The Minto-Mapleton Family Health Team continues to offer smoking cessation services at both our Clifford and Drayton locations.

    Our staff is well qualified to help you with smoking cessation. The pharmacist is a certifiedTEACH clinician (
    Training Enhancement in Applied Cessation Counseling and Health) and has considerable experience with smoking cessation counseling. Our Health Promoter and one of our Nurse Practitioners have completed training at the University of Toronto on Essential Strategies for Tobacco Cessation in primary care with TEACH. Together, they are running the Family Health Team's smoking cessation program.

    The Family Health Team is now participating in the 
    "STOP with FHTs" study, which is a Ministry of Health and Long Term Care funded project that will continue until the Spring of 2013. The project is evaluating the effectiveness of offering counseling support and free nicotine replacement therapy (Nicoderm patch, lozenge, inhaler or gum).

    Congestive Heart Failure/Heart Function Clinic
    The Family Health Team has partnered with St. Mary's Regional Cardiac Care Centre (Heart Function Clinic) to assist with the management of heart failure patients, address overall patient satisfaction and improve quality of life.

    The Family Health Team's Chronic Disease Management and Prevention Registered Nurse, Dietitian, Pharmacist and Health Promoter/Exercise Specialist work together with your Primary Care Provider and St. Mary's Heart Function Clinic staff. Our goal is to assist hear failure patients to better manage their health.

    Diabetes Clinic
    The Minto-Mapleton Family Health Team, in collaboration with Centre and North Wellington Diabetes Network, is now seeing stable patients with Type 2 Diabetes. The program has been designed to provide patient care according to the Canadian Diabetes Association Guidelines. A team, consisting of the Registered Nurse, the Registered Dietitian and the Exercise Specialist, meets with scheduled patients on Tuesdays in the Drayton office and on Thursdays in Clifford.

    We also offering group education and individual counseling for patients diagnosed with 
    pre-diabetes to assist with positive lifestyle changes.
