What is an Ontario Health Team (OHT)?
In 2019 the Government of Ontario introduced a new model for health care called
the Ontario Health Team. Ontario Health Teams are a new way of organizing and delivering health care services that aim to create a more connected and cohesive experience for patients, families and caregivers. The goal of the Ontario Health Team is to make the system easier to navigate and easier to access by strengthening local services, improving after hours services and community supports.
The Ontario government has currently established 24 Ontario Health Team’s from the first round of applications. Rural Wellington is part of the second round of applications for an Ontario Health Team.
Health Care in Rural Wellington
Health Care in Rural Wellington versus health care in urban centres is unique in that
our physicians provide care not only in their offices, but also in long-term care facilities, an after-hours clinic, the Emergency Department and throughout our hospitals. The challenges in our rural area are different in that we have transportation barriers (no public system, cost, distance to specialists), isolation, unstable or unaffordable housing issues and food insecurity. These, when combined with the cost of services and health care aids such as medications are barriers to good health. The Rural Wellington Health Team partners are committed to working together to improve the health care system and health outcomes for residents and patients in Rural Wellington.
