The Minto-Mapleton Family Health Team continues to participate in the Ministry funded “STOP with FHT’s” program. This program is evaluating the effectiveness of offering counseling support and free nicotine replacement therapy (NRT) to our patients. Funding for this program will continue until the Spring of 2013. Available NRT include the Nicoderm patch, lozenge, inhaler or gum.
Since beginning this project in August, 2011, we have seen 90 patients enroll and our success rate remains quite high. Our smoking cessation program also includes access to Champix and Wellbutrin (Zyban).
Linda Wright has been involved with this program since November. “My experience thus far with this program has been a very good one. What makes this program good?
o positive, informative counselors
o updated information on the patch system
o customize it so that it can work for you
o no cost for program or patches
I am cautiously optimistic that this will be the year I start my new life without tobacco. I have made it 10 weeks so far. If there is a will and a good back up system, I believe there is a way.”
For more information please contact the Minto-Mapleton FHT office at 519-638-2110.

The Minto-Mapleton Family Health Team is pleased to announce the Wellington Reproductive Psychiatry Program. The FHT welcomes Dr. Vivian Polak who is offering reproductive psychiatry services to adult patients who are pregnant or post-partum (within the first year after delivery). This also includes men, as men get post-partum depression and anxiety disorders. Dr. Polak also sees patients who are suffering with pregnancy loss (miscarriage, abortion, stillbirth) and with fertility difficulties. Dr. Polak travels to the Drayton site once per week and is also available for consultation and follow-up using the videoconferencing system. Patients who would like to access this program must speak to their physician or Nurse Practitioner with the FHT for a referral. This program is also being offered in neighbouring Family Health Teams including the Mount Forest FHT, the Upper Grand FHT and the East Wellington FHT. This means that if you are seeing Dr. Polak by telemedicine (videoconference), we can arrange for you to see Dr. Polak from the system closest to your home. For more information, please contact the Minto-Mapleton FHT office at 519-638-2110.

Dr. Donald, his staff, and staff of the Family Health Team are starting a new Quality Improvement Project aimed at improving the management and control of hypertension. Over the next several months, our goal is to increase the number of patients who have been screened for hypertension and have a documented blood pressure recorded in their patient record. For those patients diagnosed with hypertension, our aim is to ensure that their blood pressures are controlled. If you are a patient of Dr. Donald's and interested in more information about this initiative, give us a call.
