We heard from over 800 of you and will be using this data to guide our services and programs for this coming year. Please note you will only be asked to complete a survey once a year between April 1, 2023 through March 31st 2024 and the link will be sent to you only after a visit with your primary care provider or the Family Health Team.
We heard that it is sometimes difficult to make appointments and as such, we have recenlty added on line booking with some of our providers. We heard that some of you are enjoying virtual services and we are making an effort ot offer this whn appropriate. Be sure to have your say this year. Our new survey will be rolling out this month with survey monkey and we have updated the questions. |

Exciting news from one of the MMFHT kinesiologists!
The virtual Post-Partum Education Program is being launched in Sept again in collaboration with a kinesiologist from the Brockton FHT. This program will help support mothers with the physical and emotional aspects of post-partum; including teaching them the support resources that are in the community, helping mothers with physically healing after child birth, work on returning to activity goals, helping them with the emotional post-partum, and provide education for a mother’s nutrition. |
The program is open to post-partum women between 1-12 months, and is open to the community.
This program will be a virtual one hour class starting at 11am on Wednesday Sept 14th for 6 weeks.

Cervical cancer is almost entirely preventable with the HPV vaccine, regular testing with the Pap test, and appropriate and timely follow-up of abnormal results. There is no cost for Pap tests as they are covered by OHIP. If you are between 21 and 70, a Pap test is recommended every 3 years.

November is falls prevention month! Balance exercises are recommended to engage in twice a week for those over the age of 65. Exercise is a great way to maintain one's independence, improve coordination and more importantly improve reaction time or reflexes. All of these things are important to help you avoid falls and to help you recover from a slip!
Are you at risk of falling? Bone health is important in falls prevention. If you are over 65, talk your health care provider about having a bone mineral density test. Learn more here. |

Information Regarding the 2021-2022 Influenza Vaccine:
We must not delay vaccination to wait for a particular vaccine.
Flumist is not publicly funded for 2021-2022 influenza season.
- Anyone over the age of 6 months can receive a Quadrivalent Standard-Dose vaccine (4 strains which includes 2 influenza A strains and 2 influenza B strains)
- Those 65 years and older should receive either the:
- Quadrivalent High-Dose (2 strains of influenza A and 2 strains of influenza B at higher doses than the standard dose) OR
- Adjuvanted Trivalent vaccine (2 strains of Influenza A and 1 strain of influenza B with an adjuvant that improves immune response)

The vaccination site for our area will be the Centre Wellington Community Sportsplex in Fergus (there are other site options which the website link will show you such as Guelph and Orangeville). If you cannot come to Fergus there will be pop-up mobile sites coming to Drayton and Palmerston (we do not currently know when this will be). Physician Charity Softball event

On June 22nd, the Physician Charity Softball Event was held with a strong representation of north wellington physicians beating out the south! What a fantastic day of fun, food, games and of course, the ballgame. Thanks to the leadership of Dr. Christine Peterkin and strong support from the hospital Foundations, staff and volunteers, the event was a big success. All proceeds from the day go to the hospitals. It was great to see many physicians participate and bring their families out to cheer and enjoy the afternoon.
Do you or someone you know have an interest in changing substance use behaviours? The MMFHT provides counselling for individuals, as well as support to friends and family members that are concerned about their loved one’s substance use.
If you would like to access this service speak to your family Doctor or Nurse Practitioner and consent to a referral. Be sure to have an open mind and be ready to actively participate in goal setting to change current substance use behaviours.

The Minto-Mapleton FHT and Mapleton Health Centre are offering flu shot clinics in Palmerston, Clifford and Drayton. These clinics are open to the public. You do not need an appointment, just bring your valid health card and wear a short sleeve shirt. The MMFHT Social Workers have been designing skill building workshops to teach our patients practical tools and share tips on managing a number of every day stressors. These workshops are 1.5 hours and are offered at either our Palmerston or Drayton offices. The initial workshops for Anxiety management and Depression management have been well attended this year and will be continuing this fall. New for this fall are workshops to help patients deal with postpartum mood disorders and another on relationship building for couples. COPD is short for "Chronic Obstructive Pulmonary Disease", and it’s the new name for emphysema and chronic bronchitis. Some non-smokers can also get COPD. Susan McAuslin, Physiotherapist (PT) with the Arthritis Society is going to be offering services from our Palmerston site 2 days a month. The Arthritis Rehabilitation and Education Program (AREP) is a community-based rehab service for people of all ages with arthritis.
AREP provides a range of services including consultation and guidance on arthritis self-management; group education sessions and individual arthritis service plans. The AREP PT can assess and help with management of symptoms, gait, mobility issues, exercises to improve strength, endurance and flexibility. The PT can also recommend assistive devices and provide links to other healthcare or community services. Services are at no cost to clients with a valid Ontario health card number. GLA:D Canada is an 8-week education and exercise program for those with stiff and /or painful knees and/or hips, or those with knee and/or hip osteoarthritis. Participants report less pain, reduced use of pain medications, fewer individuals on sick leave, and being more physically active.

The FHT Social Workers have developed some workshops/skills groups that will be offered to patients on an ongoing basis. It is our hope that this will provide patients with some quick tools that they can use when things are tough and while they wait for an appointment.
At this time 3 modules have been created and will be presented in a rotation at alternating locations: Stress Management Workshop: Learn practical skills for coping and managing life’s stressors. Runs 1:30-3 pm in Palmerston February 8; or Drayton March 22. Dealing with Depression Workshop: Learn more about depression and practical tools to help. Runs 1:30-3 pm in Palmerston March 8; or Drayton April 19. Anxiety 101 Workshop: Learn practical tips and skills for coping with anxiety. Runs 1:30-3 pm in Palmerston April 5; or Drayton February 22. We have been open for our first month at our new centre in Palmerston. We are thrilled with our new space and pleased to be able to offer programming in a more central location. Staff are not the only ones happy about the new space. There have been over 100 patient visits to our activity room in May. Comments from patients include: “The NuStep (pictured here) is great, super!” and “Thank you for providing a fabulous Physical Activity Room at Palmerston hospital. I am a two time stroke survivor with physical impairments and find the well equipped room most helpful for my recovery. Sandy Turner, the coordinator, is most helpful, positive and friendly when directing the exercise programs.”

On May 4th dignitaries and community members gathered to celebrate the opening of the new Minto Rural Health Centre (MRHC) - an 11,750 square foot Medical Arts Building.
The building houses a number of key community partners in the delivery of health services to area residents. Tenants include the Palmerston and District Hospital Foundation; local Family Physician, Dr. Christopher Cressey; Minto-Mapleton Family Health Team; Canadian Mental Health Association; St Elizabeth’s Wound Clinic and Community Care Access Centre. Additional community services can be accommodated as well as additional primary care physicians.
The MRHC is located beside Palmerston and District Hospital (PDH) allowing easy access for patients to flow to and from the hospital. The reception area for the building provides a large, bright atrium allowing natural light to flow throughout the health care centre. Financing for the building came from three sources – PDH Foundation, North Wellington Health Care (NWHC) and a debenture that NWHC arranged with the Town of Minto.

The Minto-Mapleton in motion committee partnered with 4 local schools to play in motion Bingo. The bingo cards challenge and encourage students and their families to be more active and healthy. On Tuesday, April 28, 2015, the Celebratory Ground Breaking Ceremony was held for the new Minto Rural Health Centre in Palmerston. The new Centre will be built on the Palmerston and District Hospital property and open for business in 2016. Enhancing Access to Care for North Wellington Residents
Residents in North Wellington will benefit from increased access to physiotherapy care and a new integrated health centre, MPP for Guelph Liz Sandals announced this morning.
As part of reforms to physiotherapy services, Ontario is expanding access to clinic-based physiotherapy for seniors and eligible patients in North Wellington. The province is providing $31, 200 in annual funding to North Wellington Health Care to provide outpatient based physiotherapy services. This new funding will provide access to physiotherapy for approximately 100 residents each year.
North Wellington Health Care also received clearance from the Ministry of Health and Long-term Care to proceed with the planning of The Minto Rural Health Centre (MRHC) - an 11,000 square foot Medical Arts Building proposed for the Palmerston and District Hospital property.
The building will house a number of key community partners in the delivery of health services to area residents. This includes Minto-Mapleton Family Health Team, Canadian Mental Health Association – Waterloo Wellington District, Waterloo Wellington Community Care Access Centre, along with a number of primary care physicians. Additional community services can be added.

Residents in North Wellington will benefit from increased access to physiotherapy care and a new integrated health centre, MPP for Guelph Liz Sandals announced this morning.
As part of reforms to physiotherapy services, Ontario is expanding access to clinic-based physiotherapy for seniors and eligible patients in North Wellington. The province is providing $31, 200 in annual funding to North Wellington Health Care to provide outpatient based physiotherapy services. This new funding will provide access to physiotherapy for approximately 100 residents each year.
North Wellington Health Care also received clearance from the Ministry of Health and Long-term Care to proceed with the planning of The Minto Rural Health Centre (MRHC) - an 11,000 square foot Medical Arts Building proposed for the Palmerston and District Hospital property.
The building will house a number of key community partners in the delivery of health services to area residents. This includes Minto-Mapleton Family Health Team, Canadian Mental Health Association – Waterloo Wellington District, Waterloo Wellington Community Care Access Centre, along with a number of primary care physicians. Additional community services can be added.

The Rural Wellington Collaborative Mental Health Services began February 13, 2014 between CMHA-WWD and the Rural Wellington Family Health Teams. These sites include Mt. Forest, Minto-Mapleton, East Wellington and Upper Grand. The Psychiatrist, nurse, and manager of CMHA WWD attended a Meet & Greet session with the local physicians on June 17th, 2014 at the Palmerston & District Hospital.

New Health Link to Co-ordinate Care for Seniors and Patients with Complex Conditions

Residents in rural Wellington will soon benefit from better co-ordinated care from a new Health Link bringing together service providers in the community. This is the second Health Link in Waterloo Wellington, with the first underway in Guelph and two more coming soon in other areas.
The new rural Wellington Health Link will offer seniors and patients with complex conditions better care through personalized care plans and more co-ordinated support from a tightly knit team of providers. It will help improve patient transitions within the health care system by developing ways for providers to share patient information and work together to address the patient’s specific needs.
Providing better care for seniors and patients with complex conditions supports the government’s Action Plan for Health Care and its commitment to provide the right care, at the right time, in the right place. This is part of the Ontario government's economic plan to invest in people, invest in infrastructure and support a dynamic and innovative business climate.

- Since the program’s launch in December 2012, 37 Health Links have been established across Ontario.
- Health Link patients typically have complex conditions, including, multiple chronic diseases, and mental illness and addictions.
- Health Links bring together health care providers in a given geographic area, including primary care providers, specialists, hospitals, home care, long-term care and community agencies.
- A recent study found that 75 per cent of seniors with complex conditions who are discharged from the hospital receive care from six or more physicians and 30 per cent get their drugs from three or more pharmacies.
- Complex patients represent up to five per cent of Ontario’s population, but use two-thirds of the health care budget.
- This is the second Health Link in Waterloo Wellington. The other Health Link in our region is in Guelph, led through the Guelph Family Health Team (www.guelphfht.com) .

Our local Minto-Mapleton Communities in Motion Committee has recently donated Daily Physical Activity Kits to a number of local schools in our area, including Drayton Heights, Maryborough Public School and Palmerston Public School. These kits are filled with DVD’s, CD’s and numerous activities to ensure our kids reach the recommended 60 or more minutes of physical activity a day! Here, our Health Promoter, Sandy Turner is delivering the kit to the Grade 5/6 class at Maryborough public school. The schools will also be participating in the in motion BINGO challenge next week for a chance to win prizes that promote being in motion!

The Minto-Mapleton Communities in Motion (MMCiM) committee donated over 40 pedometers to our local library branches, including the Drayton, Palmerston, Harriston and Clifford sites. Promoting physical activity is the core of the MMCiM and many projects have been aimed at youth, so, the pedometer initiative was a good way to get adults active as well. The pedometers are available for loan much the same way books are at the library. You will also have access to an on-line tracking site to set daily step goals, if you wish. For health, it is important to aim for 10,000 steps a day, so take the challenge and wear a pedometer today!

The Minto-Mapleton Family Health Team attended the 32nd Annual Drayton Farm Show. They joined over 100 exhibitors and held a successful blood pressure clinic screening over 250 people. They also provided delicious quinoa salad samples to many visitors and vendors - what a hit! This was the team’s third year attending the Farm Show and the goal was to reach community members that do not visit their health care provider on a regular basis to screen for hypertension. They also provided information and handouts on ways to prevent and control hypertension. Three staff of the Minto-Mapleton FHT (Paula Seifried, Shirley Borges and Kelly Burrows) have just completed LEAN Training in conjunction with staff from neighbouring FHTs and hospital staff from Groves Memorial Community Hospital and North Wellington Health Care Corporation. LEAN training is designed to give participants tools and processes to solve problems with the goal of creating “problem solving organizations”. Upon completion of training, each participant identifies a problem in his/her workplace and applies the LEAN tools and methods to fix it. The Minto-Mapleton FHT is proud of its first LEAN participants and looks forward to future updates.

On Dec 5, 2012 the Minto-Mapleton Family Health Team hosted the Annual CHF Christmas Luncheon for 23 diners at the Palmerston United Church.
The delicious meal was planned, prepped and presented by our registered dietitian, Paula Trinier. A special thanks to Sandy Turner, our heath promoter, who assisted in the festivities of the day; and volunteer Jessica Schill from Norwell District Secondary School.

Our FHT has been working with over 100 of our patients in the past year to help them reduce or quit smoking by offering counseling services and FREE Nicotine Replacement Therapy (NRT) in the form of patches, gum, inhaler and lozenge. This program has just been extended for another year and will be concluding March 2014! Presently, we are offering this service at our Drayton and Clifford offices. Dermatology consults are now available. The telemedicine nurses will take photos of the area needing consultation and will forward them to a dermatologist for their opinion. Turn around time can be as quick as a few days. See your doctor with dermatological questions to see if you are a candidate for this process. This can save months of waiting for an in-person appointment, the drive to the city, and extra unneeded time off work.
We are in the planning phases of new programs including Osteoporosis, Child and Adolescent Psychiatry, and Nephrology. Stay tuned for more updates in the near future!

We are excited to have two new positions at the Minto-Mapleton Family Health Team! Pam Turton, RN, and Tracy Livingston, RPN, will be working collaboratively with the Minto-Mapleton and Mount Forest Family Health Teams and North Wellington Health Care (Palmerston and Mount Forest hospital sites) to develop and deliver the Telemedicine Program for north Wellington. Telemedicine involves using secured two-way videoconferencing through the Ontario Telemedicine Network (OTN) that connects patients with specialists and other health care providers. The Ontario Telemedicine Network is one of the largest telemedicine networks in the world. This technology will give our local patients access to many different types of specialists who ordinarily would be one to several hours drive away. Examples of such specialties include: psychiatry, dermatology, general surgery (for pre- and post-op appointments), pediatrics, and other specialties. We are excited to offer Craving Change: a how-to workshop for changing your relationship with food. This is a program for people who struggle with their eating and want to feel more in control over what and how much they eat. Our goal is to help you understand WHY you eat the way you do and to provide tools to help you comfort yourself without food. Craving Change starts June 28th in Drayton.
Our second new program, Tastes for Life, was developed by the Mount Forest FHT in partnership with the Seniors Centre for Excellence. It is a free 6 week cooking and healthy living program for seniors or those cooking for 1 or 2 people. Topics include nutrition, cooking skills, meal planning, and active living. At each class we will go through 4-6 different recipes to give you tips and tricks to help you make healthy meals quickly and effortlessly while at the same time not compromising on taste. There are also lessons on ways to get active including using pedometers, thera-bands and other safe at home activities. This program will held in Palmerston and begins July 11th.
Join us for Colour It Up! This group is also being held in Palmerston starting July 23rd. It is a free 6 week program for women with children ages 6 and under. Each session will include discussions and hands-on activities to help get you and your family eating more fruit and veggies! The Early Years Centre will be providing FREE childcare for the sessions.
We will also be holding monthly Grocery Store Tours. Do you find grocery stores overwhelming? There is so much nutrition information out there it may make grocery shopping seem like a scavenger hunt! During our tours you will learn to quickly read and understand nutrition labels and make the healthiest and delicious choices possible, even on a budget! Come out and have your chance to ask questions about things you have heard from friends or in the news!

The Minto-Mapleton Family Health Team continues to participate in the Ministry funded “STOP with FHT’s” program. This program is evaluating the effectiveness of offering counseling support and free nicotine replacement therapy (NRT) to our patients. Funding for this program will continue until the Spring of 2013. Available NRT include the Nicoderm patch, lozenge, inhaler or gum.
Since beginning this project in August, 2011, we have seen 90 patients enroll and our success rate remains quite high. Our smoking cessation program also includes access to Champix and Wellbutrin (Zyban).

Representatives of the Rural Wellington hosptials, Family Health Teams in Wellington, the Community Care Access Centre and Trellis Mental Health and Developmental Services formally signed an agreement or Memorandum of Understanding aimed at working together in partnership to improve health services in our rural communities. The partners have created a working group called the "Rural Health Care of Tomorrow". The Working Group looks forward to keeping the community updated on its progress.

The Family Health Team (FHT) launched a new group this spring called Chronic Disease Self-Management Program where patients of the FHT are introduced to a useful self-management tool with the goal of increasing healthy behaviours. Shown in the picture are: high school co-op student Caitlyn Pirker, Paula Trinier, Registered Dietitian, Vicky LaForge, Registered Nurse and a few of our Program participants. Dr. Donald, his staff, and staff of the Family Health Team are starting a new Quality Improvement Project aimed at improving the management and control of hypertension. Over the next several months, our goal is to increase the number of patients who have been screened for hypertension and have a documented blood pressure recorded in their patient record. For those patients diagnosed with hypertension, our aim is to ensure that their blood pressures are controlled. If you are a patient of Dr. Donald's and interested in more information about this initiative, give us a call. Looking for a Primary Care Practitioner? Nurse Practitioner, Jodi Colwill, working in collaboration with Dr. Christine Peterkin, is now accepting patients at the Mapleton Health Centre. Anyone who does not have a family physician in Minto-Mapleton and who is seeking the services of a nurse practitioner, should call the Mapleton Health Centre in Drayton at 519-638-3088. Beginning in September, 2010, Jodi will begin seeing new patients at the Drayton site.

The Minto-Mapleton FHT has been approved as one of 20 Physician Assistant Demonstration Projects in Family Health Teams across the province. The project is a two year pilot aimed at determining how Physician Assistants can best integrate into primary care. Once recruited, the Physician Assistant will work under the supervision of Dr. Michael Kam at the Mapleton Health Centre. The FHT has until March 31, 2010 to recruit a certified Physician Assistant. A comprehensive evaluation strategy is part of this new funding initiative.
