    EVENTS AND WORKSHOPS

    Chronic Disease Self Management Program

    The Minto-Mapleton Family Health Team is encouraging our patients with Chronic Disease to become confident self-managers.

    We offer a 6-session program to assist in developing the skills required to manage your Chronic Disease. This group setting is ideal for those with chronic conditions sharing similar concerns and problems. Active participation is encouraged. Participants are taught healthy behaviours, symptom management, coping, and communication and participate in practicing these tools.

    Chronic Pain Self Management Program

    This is a 6-week group for patients dealing with chronic pain. Patients attend once a week for a 2.5-hour education class. Some topics include fitness/exercise, pacing, medications, emotions, fatigue/sleep and communication. This program is geared at teaching new ways to deal with chronic pain and requires participants to take an active role in learning and practicing these tools. The support from the group members is an added benefit to the program. A family member is welcome to attend as well.

    Grief Group

    The social workers of the Minto-Mapleton FHT offer an 8-week grief group. This group provides an opportunity to better understand the grieving process as well as to connect with others who have experienced loss.

    Cholesterol Class

    The dietitian and health promoter with the Family Health Team offer a 2-hour education class for patients with elevated cholesterol. During this interactive class, patients will learn healthier choices around diet and exercise that will positively affect their cholesterol profile and overall health. Monthly classes are offered in the afternoon and evening.

    Walking Groups

    The Minto-Mapleton Family Health Team (FHT) continues to partner with the Seniors' Centre for Excellence to provide indoor walking groups in Harriston, Palmerston, Clifford and Drayton. The township of Mapleton and the Town of Minto have graciously offered the Community Centres in each of these towns to accommodate our morning walking clubs for FREE! We have identified a walking champion in each community. The role of the champion is to help maintain the group, get new people started and act as a liaison between the participants and the Minto-Mapleton FHT's exercise specialist, Sandy Turner. Each walking club offers walking from October through April, and the Drayton and Palmerston clubs continue throughout the summer months as well, walking on the ice surface at the arena.

    If you are an active walker during the summer months but find it difficult to maintain this throughout the year, then our walking groups may be just the answer. It's a great place to meet new people, and keep active at the same time!

    If you are just starting a walking program, the champions are able to help you get started. Just give us a call and we will set up a start date for you to get on your way to walking to better health!

    Locations and Walking Times: (October through the end of April)
    Palmerston Community Centre*: Monday, Wednesday and Friday @ 9:00-10:00 am
    Drayton Community Centre* (PMD Arena): Tuesday, Thursday and Fridays @ 9:00-10:30 am
    Harriston Community Centre and Clifford Community Centre:  There has not been enough interest to sustain a walking group in this community. We are hopeful that this will change in the future if we are able to generate more interest. If you are interested in joining this group please contact us at either number listed below. Once a minimum number of participants have been identified we will commence the walking group.

    Cognitive Behavioural Therapy (CBT)

    The social workers of the Minto-Mapleton FHT offer Cognitive Behavioural Therapy (CBT) groups throughout the year. The focus of the group is to introduce concepts with a focus on developing skills to better manage symptoms related to anxiety and depression. The groups typically run on Tuesday afternoons and begin in the fall, winter and spring.

    The positive and enthusiastic response to our CBT groups has resulted in the addition of monthly booster sessions. Completion of an 8 week CBT group is the only pre-requisite to attend the booster group which focuses on further skill development and an opportunity to connect with other participants on a regular basis.

    Learn to Run

    The Family Health Team's Exercise Specialist, Sandy Turner, runs spring and fall Learn to Run Groups in both Harriston and Drayton.

    This is a 10 week program where participants meet once a week for a gradual, progressive walk-run routine, with 2-3 nights of additional "home-work."

    Each week will also bring new education topics that include:
    »stretching basics
    »breathing techniques
    »food and hydration
    »staying in shape all year

    What will you get out of the running group? At the end of this 10 week program you will be able to run continuously for 30 minutes, and this group will be participating in a local 5km run.
