mental health therapist - permanent, full time

The Minto-Mapleton Family Health Team (MMFHT) offers the opportunity to join an innovative, community-based primary health care team serving Drayton, Palmerston, Harriston and Clifford areas. Due to a retirement, we are looking for a talented team player to join us in our Drayton Office. 
Position SummaryThe Mental Health Therapist (MHT), as a member of a multidisciplinary primary care health team,  provides therapy to patients with mild to moderate mental health issues through individual and group counseling and is a resource to family physicians, other allied health professionals and the community. The MHT will provide counselling/ therapy /treatment plans designed to help the individual to perform daily tasks, and self-management coaching and support to develop the skills to live independent and productive lives.
Requirements
  • Master of Social Work, preferred, and current registration in good standing with the College of Social Workers
  • Ability to work independently and as part of a collaborative team
  • Prior experience in assessment and counseling in a primary care setting is considered an asset
  • Program development, facilitation and evaluation skills
  • Strong leadership, interpersonal, problem-solving and organizational skills
  • Interest or experience in teaching and mentoring students
  • Ability to prioritize, manage time effectively and be flexible within a dynamic work environment
  • Excellent oral and written communication skills
  • Access to method of reliable transportation
  • Knowledge and proficiency in the use of computer and communication systems including e-mail and electronic medical records (EMRs)
  • Up-to-date immunizations including COVID

Family Physician

The Minto Mapleton Family Health Organization (FHO) is currently recruiting a Family Physician to join our group.  This is a new position.  Minto Mapleton is a rural community that includes physician office locations in Drayton, Palmerston, Harriston and Clifford (35-45 min from Kitchener-Waterloo and Guelph).  

The team currently consists of 9 physicians providing office and inpatient care.  Many of the team also provide care to LTC patients, ER and OB/delivery.  ER shifts are 24 hrs or can be split to 12 hour shifts if preferred and remunerated via an AFA.  OB on call is also available with remuneration via HOCC.  There are opportunities to work in ER, long term care, anesthesia as well as surgical assist.

The Minto Mapleton FHO is lucky to also have a Family Health Team (FHT) associated with it with a number of health professionals including Nurse Practitioners, Physician Assistant, Registered Dietitian, Social Workers, Registered Nurses, Health Promoter, Pharmacist as well as an elaborate telemedicine program. 

The Palmerston & District Hospital is part of the North Wellington Healthcare Alliance (NWHA), which is an Alliance of 2 hospitals that includes Palmerston & District Hospital and Louise Marshall Hospital.  Palmerston & District Hospital has 15 inpatient beds and includes a full time ER department, laboratory, diagnostic imaging and Obstetrics/Gynecology.  Attending specialists include internal medicine, urology and endocrinology as well as surgical services including gastroenterology, OB/GYN, general surgery and otolaryngology.

Physicians agreeing to practice Full Time comprehensive medicine, will be eligible to apply for provincial funding through the Northern Rural Recruitment and Retention Initiative in the amount of $81,200.00 over 4 years.
