    FHT Board of Directors

    The Minto-Mapleton FHT was incorporated in October, 2006 and the first staff were hired in December, 2006. The FHT is physician-directed and as of June, 2007, all eight (8) of the physicians in the Minto-Mapleton are directors of the FHT. Dr. Chris Cressey (Palmerston) is Medical Lead for the FHT. These eight (8) physicians have approx. 12,000 patients in total. The following physicians are directors of the FHT Board:

    Dr. Gavin Semelhago, Treasurer and Director (Palmerston)
    Dr. John Vanderkooy, Director (Harriston)

    The FHT Board holds monthly meetings on the third Tuesday of each month at the Palmerston and District Hospital.



    FHT Mission

    The Minto-Mapleton FHT is a collaborative, interdisciplinary primary care team located in Minto (Palmerston, Harriston and Clifford) and Mapleton (Drayton). Our mission is to efficiently provide high quality, accessible, patient-centred care that offers a comprehensive range of core and targeted services aimed at managing and preventing disease and improving health status.


    FHT Guiding Principles

    The work of the Family Health Team (FHT) is guided by the following principles:

    » Team-based care: We work in interdisciplinary teams of providers including physicians, nurses and other health care professionals to meet the needs of our patient population.

    » Community partnerships: We work in collaboration with our community partners including other health care organizations and professionals.

    » Realistic and Responsive: We strive to respond to the changing needs of our patients in a realistic and timely manner when we make decisions about FHT programming and services.

    » Evidence-based practice: We are committed to developing and using evidence-based practice and continuous re-evaluation to guide our efforts and measure our results.

    » Innovation: We use creative solutions that respond to local community and provider needs to achieve the FHT objectives.

    » Research and education: We value and utilize research and education opportunities provided through the FHT. 

    » Patient-centred: We involve our patients in decisions affecting their care and strive to find solutions that are acceptable to them. It is through patient-centred care that we promote a culture of mutual trust and accountability.



    FHT Objectives

    It is the intent of the Minto-Mapleton FHT to achieve the following patient-centred strategic objectives by providing the full spectrum of core/comprehensive FHT services as well as programs targeted to specific population needs.

    » To enhance the provision of primary care core services to our catchment population.

    » To increase patient accessibility to interdisciplinary primary care services.

    » To increase multidisciplinary approaches to patient care and decision-making.

    » To increase access to primary care services for hard to serve groups/patients.

    » To build on and enhance linkages with our key community partners.

    » To increase the emphasis in primary health care on chronic disease management and health promotion and preventative care.
